The Phoenix Rehabilitation and Nursing Center Data Breach Details
Phoenix first became aware of the breach on or around September 19, 2024, and has since conducted a thorough investigation to determine the scope of the incident. According to information from the vendor, the breach occurred on or about July 20, 2024. Upon identifying the affected data set, Phoenix initiated efforts to notify those potentially impacted.
Recognizing the growing frequency of cyberattacks, Phoenix stated it is taking additional steps to strengthen its security measures. “Unfortunately, these types of incidents are becoming increasingly common, even for organizations with sophisticated IT infrastructure,” the company said in a statement.
To inform those affected, Phoenix mailed formal notification letters on January 28, 2025. These letters provide further details about the breach, outline the specific data that may have been compromised, and include resources for those seeking more information.

As of now, Phoenix has not received any reports of identity theft, fraud, or misuse of affected data.
