Affected individuals are urged to stay vigilant by regularly reviewing their personal information and monitoring their credit reports for any suspicious activity.

Alternate Solutions Health Network Data Breach Details

The breach occurred when an unauthorized actor accessed an ASHN employee email account on or around May 30, 2024. The compromised account contained sensitive personal and health information, including names, dates of birth, addresses, driver’s license numbers, physician and clinician names, clinical information, diagnostic information, treatment information, and, in some cases, Social Security numbers.

Upon discovering the breach, ASHN secured the affected email account and initiated an investigation with the assistance of external cybersecurity professionals. The investigation determined that the unauthorized access occurred between May 30, 2024, and February 14, 2025. ASHN reported the breach to the U.S. Department of Health and Human Services on April 14, 2025.

Compromised Information

The compromised data may include personal details such as:

  • Full name
  • Address
  • Phone number
  • Email address
  • Date of birth
  • Social Security Number
  • Treatment and diagnosis details
  • Prescription information
  • Provider name
  • Medical record or case number
  • Health insurance information

While the investigation is ongoing, there is currently no evidence to suggest that the compromised information has been misused.

How to Know if You Were Affected

If you were affected, you should have received a direct notification from Alternate Solutions Health Network, such as a letter or email. While most victims are officially notified through a “Notice of Data Breach” in the mail, the consequences—such as fraudulent transactions and identity theft—can begin before the official notice arrives.

What You Can Do to Protect Your Information

If you believe you were affected by the breach, there are several steps you can take to protect your personal information:

  1. Monitor Your Credit Reports: Review your credit reports for any suspicious activity or errors. You are entitled to one free report per year from each of the three major credit reporting bureaus—Equifax, Experian, and TransUnion.

  2. Activate Free Credit Monitoring: Enroll in the free credit monitoring services offered by Credit Sesame through Experian. The service includes:

    • Daily credit report updates

    • Identity theft insurance coverage up to $1 million

    • Identity Restoration services for resolving fraud issues

    • Additional fraud protection features

  3. Place a Fraud Alert or Credit Freeze: You can place a fraud alert or a credit freeze on your credit file with each of the three bureaus to prevent unauthorized access.

  4. Create Strong Passwords and Use Multi-Factor Authentication (MFA): Strengthen your security by using complex passwords for your accounts, avoid reusing passwords across multiple sites, and activate MFA wherever possible.

  5. Watch for Phishing Attempts: Be wary of phishing attempts, especially those that may claim to be from Alternate Solutions Health Network or other institutions asking for personal details.
