Claxton-Hepburn Medical Center Data Breach Details
The breach occurred on August 31, 2023, and was discovered on September 6, 2023. Upon learning of the issue, CHMC initiated an immediate and thorough investigation, engaging third-party cybersecurity professionals experienced in handling such incidents.
The investigation revealed that certain files within the hospital’s network contained individual personal and health information. The compromised data includes names, addresses, dates of birth, Social Security numbers, medical information, health insurance information, and medical diagnosis information.
CHMC has notified affected individuals via letter and is offering complimentary credit monitoring services to eligible individuals. The hospital advises individuals to remain vigilant by regularly reviewing financial account statements for any fraudulent activity and to review explanation of benefits statements for any unrecognized items.
Individuals have the right to place an initial or extended fraud alert on a credit file at no cost. If individuals are victims of identity theft, they are entitled to an extended fraud alert lasting seven years. As an alternative to a fraud alert, they have the right to place a credit freeze on a credit report. The credit freeze is designed to prevent credit, loans, and services from being approved without consent.
