Hopscotch Primary Care (formerly known as Cannon Family Health) is notifying patients of a recent security incident that may have compromised the privacy of certain protected health information (PHI). The breach involves a limited set of legacy paper records from the time when Cannon Family Health operated under that name.

What Led to the Hopscotch Data Breach?

On August 27, 2024, Hopscotch became aware of a data security incident in which unauthorized individuals gained access to physical paper records from Cannon Family Health. While the incident was isolated to these physical records, Hopscotch took immediate action. Upon discovering the breach, the organization quickly coordinated with the Buncombe County Sheriff’s Office and engaged law enforcement for assistance. As a result of these actions, the suspect was detained and is facing prosecution.

Though the breach involved legacy paper records, Hopscotch assures its patients that its electronic medical record system, which houses current patient records, was not affected by this incident. It’s important to note that Hopscotch’s electronic medical record system, which houses all active patient treatment records, was not impacted by this incident. To ensure transparency, written notifications were sent to 4945 Illinois residents.  Impacted individuals are strongly encouraged  to monitor their financial accounts for any unusual activity.

On September 19, 2024, law enforcement granted Hopscotch access to a portion of the compromised records. The investigation revealed that certain personal health information (PHI) from these legacy paper files may have been exposed. This potentially impacted information includes:
