On October 9, 2024, Mutual of Omaha Insurance Company completed an update to its printing software. Unfortunately, this update inadvertently included a coding error that resulted in the mismailing of a small portion of policy packets. Upon discovering the error, Mutual of Omaha promptly corrected the code and initiated a thorough investigation to assess the extent of the impact. On November 21, 2024, the investigation confirmed that your insurance application was mistakenly included in another policyholder’s policy packet.

Although there is no indication that your personal information has been or will be misused as a result of this incident, we recommend remaining vigilant in safeguarding your information. This includes monitoring your accounts and credit reports for any unusual activity. Mutual of Omaha has also arranged for Experian to provide you with complimentary identity protection services for 12 months. These services include credit monitoring, fraud detection, and identity restoration support.
